Due to the nature of our practice, emergencies do arise which may cause unavoidable delays or the need to reschedule your appointment. In such instances we will do our best to notify you in advance. Thank you for your patience and understanding.

| Please bring the following items to your first appointment: |

| • A copy of all pertinent medical records |
| • A list of all current medications including non-prescription and over-the-counter |
| |
medications |
| • Current insurance card and referral form (authorization) if applicable. Patients who are |
| |
members of an HMO plan will not be seen without an authorization |

The initial consultation visit will take approximately 30 minutes. Follow up visits are usually 15-30 minutes. Please arrive for your initial visit 15 minutes before the appointment time to fill out necessary forms.

If you need to cancel your office appointment please call 24 hours in advance to avoid a late cancellation fee. If you are canceling your procedure, please call to cancel 3 business days in advance to avoid a late cancellation fee. Our answering service will take a message when the office is closed.

If you have any question regarding your gastrointestinal or liver condition, please phone our office during office hours. Our staff is specially trained to take your information accurately and notify your doctor. Please leave both daytime and evening phone numbers as your call may be returned after office hours.

General questions that are not related to your gastrointestinal or liver condition should be directed to your primary care physician.

In case of an emergency, please call 9-1-1, or go to the emergency room of the nearest hospital. The physician on duty will initiate treatment and notify our on-call physician. |

If you have an urgent problem, please call our office. Your doctor or the on-call physician will be notified of your problem. If the office is closed, the answering service will take pertinent information and notify our on-call physician.

Please check your medications regularly to make sure you do not run out. If a refill is needed, please call the pharmacy. If there is any question regarding the prescription, the pharmacy will contact our office. Refills requested after office hours and on weekends will be approved the next working day.

Charge for initial consultation, follow-up visits, hospital visits and procedures are determined by the severity and complexity of the problem. Please do not hesitate to discuss our fees if you have any questions. It is always wise to anticipate the cost of services.

You will receive an itemized statement displaying a description of office or hospital charges and any payment credited to your account. If you have insurance, you are responsible for notifying our business office of your current carrier. We will then extend the courtesy of billing your insurance company for you. |

Please be aware that even though we call your insurance company to verify coverage, this will not guarantee payment by your carrier. You will be responsible for any remaining balance. |
